What is esophageal cancer and its treatment : The esophageal cancer disease hits adults, and the likelihood of its occurrence multiplied with the passage of time from the age of 50 and above, which is more common in men. The primary types of this type of cancer are the squamous cell and adenocarcinoma cells.  

The incidence of adenocarcinoma, which is more common in the lower third of the esophagus, has doubled in recent decades in males of white race in the United States by 350% (from 0.7 / 100,000 to 3.2 / 100000) and the cause is unknown.

The cause of esophageal cancer is sometimes a genetic disorder in the gene called p53.

Squamous cell carcinoma usually occurs in the upper two thirds of the esophagus. Show adenocarcinoma cocculus often, in the lower third, on the basis of a change in the cells of the esophagus, following the rise of stomach acid into the esophagus (Gastroesophageal Reflux ) . Rise above the age of 50, the rate of disease by 0.5% per year for the patient.

Treatment of esophageal cancer

Treatment alternatives in such cases are continuous follow-up, surgery or photodynamic therapy, which is still being tested.

My favorite so far is surgery, but even if the entire tumor has been removed, only a small percentage is permanently cured (20% -25%).

The basics of surgery are: Tumor eradication with adjacent esophageal sections , adjacent lymph nodes, and alternative placement of the esophagus from the stomach or parts of the intestines. The access roads to the surgical area are different, so you can enter through the upper part of the abdomen and sometimes through the thorax.

In recent years, many methods of combined therapy have been tested. The results of chemotherapy were examined prior to surgery in randomized trials of hundreds of patients and their efficacy was not conclusively demonstrated. In small and concentrated tumors, we can control the disease and lengthen the lives of patients, through this treatment dramatically in 10% – 15% of cases.

The combination of radiotherapy with chemotherapy is slightly better than radiotherapy alone, but fusion with other treatments, as opposed to surgery alone, is still being studied.  

The combination of the most commonly used chemotherapy drugs for patients with esophageal cancer is fluorouracil 5 (fluorouracil), which is administered continuously, with cisplatin.

In the last decade, other effective anti-cancer drugs, such as those of the Texans and Irinotecan, have been discovered . But the effectiveness of their inclusion in the treatment is still under consideration.
